Tracking your child's developmental progress is one of the many ways that we provide quality health care for your child.

Please take a minute to register through this online link so that you will be able to access age-appropriate developmental screenings prior to your child's well-child check up.

Registration Instructions

To register you will need an invitation code. 

1. If you are a patient at PineyWoods Pediatrics, please call us at 936-560-9000 to receive this code

2. Click the following link: www.chadis.com.

3. Click "I have never used Chadis before."

4. Enter the invitation code and follow the directions to create an account for your child. 

5. In the weeks prior to your child's next check up you will receive an email with a link so you may log on and fill out the appropriate screening forms.
